Our Expectations
Please arrive 20 minutes early to your first, establishing, NEW PATIENT appointment. Future office visits can be used for complete physical exams, GYN exams, sick visits, procedures, follow up visits, forms, immunizations, blood work, injections, etc. Please bring a list of any medications you are on and/or a bag of your prescription bottles.
To save time, Print All 7 Forms now and complete them thoroughly. Also bring your actual Driver's License and Insurance Card(s) so we can make a clear copy at the office to have on file.
If you need to miss an appointment, you must call before 8 AM the day of the appointment; you may leave a voicemail phone message. Otherwise you will incur a missed appointment fee. Each slot of the day is valuable and needed by patients. We are not a Walk-In Clinic. Same day appointments are possible; however, as a courtesy to our office and our patients, please call in advance so that we can find the best time slot to Work-You-In that day.
